Fall Sports Pictures - August 13 (after school)

BEFORE PHOTO DAY

Place your order securely online by following the steps below.
Please visit https://crabtreephotos.accessmyevent.com/LapelMiddle
Choose your event and click on the link.
Choose your package, and/or add-on items, add to your cart.
Click on your cart and complete the check-out process.
During check-out, enter your player's first and last name and team name. This is how we will match the photo to your order.
You will receive an email confirmation within 24 hours of placing your order. If you do not, please call the Crabtree Photography office (765) 747-6971 to check the status of your order.
There's no need to bring any paperwork with you on photo day! Once you receive the email confirmation, we have received your order!

ON PHOTO DAY

Arrive at your scheduled photo time
Make sure you have everything you need for photo day!
Team shirt and hat, gameday pants/shoes/socks/belt, etc.
Your own sports equipment...ie. helmet, football, volleyball, etc.
If you wish to pay with cash or check, please fill out the order form completely before you arrive, and enclose exact payment inside the envelope.
(We would prefer the above online payment option, if you are able)
Order Forms available in the front office at the middle school.
Only coaches and players will be allowed in the designated photo area.

SPORTS PHYSICALS

IN ORDER TO PARTICIPATE IN SPORTS PRACTICES, WORKOUTS OR TRYOUTS, STUDENTS MUST HAVE A SPORTS PHYSICAL EXAM ON FILE (DATED APRIL 1, 2025 OR LATER), AND COMPLETED THE 2025-26 ONLINE STUDENT REGISTRATION IN PARENTVUE (INCLUDING THE ATHLETIC PARTICIPATION SECTION).

SPORTS PHYSICAL EXAMS CAN BE DROPPED OFF AT LMS, LHS OR EMAILED DIRECTLY TO TWILSON@FLCS.K12.IN.US

Add funds to your child's meal account online at linqconnect.com.
You can also send cash or check (payable to LMS) to the LMS office to be applied to meal accounts
Set up your account to be notified when your child's balance gets below a certain amount
Apply for free/reduced lunch status
View balance and transaction history
You will need your child's STN # to set up a new account - call the LMS office for assistance

Volunteer/Chaperone Information

All applications will expire and will need to be completed again (along with getting your Driver's License re-scanned) for the following school year after July 1.

FLCS is very thankful for all our volunteers that help our students and staff. If you are planning to or would like to volunteer (see examples below) at some time during the school year, you will need to complete two items prior to volunteering. (*Please note - this process needs to be repeated each school year.)

1) Please complete the online volunteer application (FLCS Volunteer Application).

2) Then, bring your Driver's License to the school office to be scanned through our Raptor Visitor Management System.

Volunteer examples include: field trip chaperone, classroom helper, athletics/coaching, library helper, cafeteria helper, tutor, office helper, fundraising, concessions, mentor, etc.
